Dear Nurse,
Re: Ms Juanita Ferreira, DOB: 13 April 1960
Ms Juanita Ferreira require further management for haemorrhoidectomy at year family which is scheduled on tomorrow.
Ms Ferreira was admitted to hospital on 12 November 2018 with rectal bleeding along with burning pain and mild tenderness. Her physical examination was performed which revealed prolapse. Consequently, she was diagnosed with great for haemorrhoids she was explained pre- procedure management including nill per oral six hours before the surgery.
During hospitalisation, advice regarding high-fibre diet and cleaning the affected area with warm water recommended. Her pain and itching were managed with Tylenol and lidocaine respectively. A proton pump inhibitor was administered for gastritis. Additionally, enemo was given and information for the post surgery management and hospital policy have been provided.
Ms Ferreira has had type 2 diabetes mellitus and inflamed bowel syndrome. She’s allergic to penicillin and aspirin.
Based on the above mentioned circumstances, further management and support would be profoundly beneficial. Please note, consent form for hemorrhoidectomy was received.
