Dear Ms Morris,

Re: Mr Joshua Gibbon, aged 27

Mr Joshua Gibbon, who underwent closed reduction and immobilization due to 5th metacarpel fracture, requires ongoing care and follow-up from your facility.

Today, Mr Gibbon presented with a fracture on the left wrist following a hit on the wall as a result of assault. On examination, numbness and pain were noted; therefore, hand was elevated. An x-ray confirmed 5th metacarpal boxer’s fracture. Consequently, he underwent closed reduction followed by plaster cast splint for immobilization. Cast care instructions were provided.

Mr Gibbon resides with mother and girlfriend. He has the history of multiple leg fractures.

Based on the aforementioned circumstances, provide continuing care for Mr Gibbon would be profoundly beneficial. A physiotherapy review at hand clinic is recommended. An appointment with a nurse practitioner has been scheduled in a week at fracture clinic with an xray.

It is essential to inform Emergency Department if you notice severe pain and numbness.

In case of any additional information is required. Please do not hesitate to contact me.

Yours sincerely,

Registered Nurse.
