Re: Mr Joshua Gibbon, aged 27
I am writing to introduce Mr Gibbon who had a boxer fracture to the left Fifth metacarpal and he requires ongoing care from your facility.
Initially, Mr Gibbon presented with a complaint of numbness on the left hand after hitting against the wall this morning during fighting with his friend. On examination, boxer fracture was suspected; therefore, an x-ray was done which revealed boxer fracture to left fifth metacarpal with displacement. As a result, the splint was attached for closed reduction and immobilisation.
Upon discharge, he has been educated on cast care as well as advised to keep his hand elevated. Please report to the emergency department if any pain and numbness to his fingers and hand. He has followed up at the fracture outpatient clinic for an x-ray and review with Nurse Practitioner in a week. He has been advised for physiotherapy at the hand clinic.
It would be appreciated if you could take our me Gibbon and provide continuing care.
