Re: Mr Jamie Morgan, 22 February 1956,
I am writing to request on behalf of Mr Morgan, who is recuperating from coronary artery bypass grafts on 29 September 2017. He requires advice on low fat dietary guidelines and healthy simple recipes following his discharge.
On 27 September 2017, Mr Morgan was admitted with thr diagnosis of obstructive coronary artery diseases and therefore, he underwent surgery successfully. Post operatively, he has made excellent progress by mobilising and wound is healing well with the help of dressing.
Mr Morgan, works as a fencing contractor lives alone in own home. He is obese and his regular diet are sausages, deep fried chips, burgers, eggs and mac donalds. In addition, he drinks alcohol and smokes 30 cigarettes per day. Consequently, he has suggested to consume low fat diet, along with to quit smoking and t reduce consumption of alcohol. It is important to note, he is allergic to peanuts. Encouragement has made to continue the rehabilitation exercise at his home with the local physiotherapist.
Based on the above circumstances, Mr Morgan, has routine visit from social worker and kindly make an arrangement for follow-up visit to or Stevenson George at 2 pm on 10 October 2017. Please note, all the needed information should be sent to his respective address. I enclosed all the relevant documents with this letter.
