Re: Mr Alfred Billy, aged 52
Mr Alfred Billy, who has been recuperating from nodular basal - cell carcinoma, requires ongoing assistance and evaluation from your service following his discharge today.
On 21 March 2010, Mr Billy was admitted with suggestive indicators of skin cancer of the neck. A skin biopsy was performed, which revealed the nodular basal - cell carcinoma. Consequently, a successful surgery was done. Postoperatively, panadeine forte 500 mg was prescribed. Currently, an excellent progress has been developed. The surgical incision shows no signs of infection.
Mr Billy works as a truck driver and resides with his wife, Maria at Arillon City Arcade 207 Murray Street Perth. He drinks alcohol for the last 30 years as well as he also smokes. No significant past medical history has been noted.
Based on the aforementioned circumstances, it would be greatly appreciated if you could provide regular monitoring to Mr Billy. Panadeine forte needs to be continued until next week.
In case additional information is required, please do not hesitate to contact me.
Yours faithfully,
