Dear Ms Jeremy,
Re: Ms Britney Wahlberg,aged 21
Ms Britney Wahlberg, a six weeks pregnant, requires further support and assistance for the management of substance abuse problem.
Ms Wahlberg resists with her friend in a rental accommodation. She smokes 30 to 40 cigarettes per day and drinks 8 to 15 units of alcohol daily for the last 15 years. She uses drugs including cannabis, cocaine, heroin and amphetamines and shares infected syringe occasionally. She has a history of depression, suicidal thoughts and self harm;however, psychiatrist appointment is not received.
Ms Wahlberg was admitted to hospital on 06 January 2019 due to concern about her pregnancy which was confirmed two days ago. Additionally, pain in lower back ,memory impairment, sleeping disturbance and hallucination have been reported.
During hospitalization, blood test was performed and counseling regarding the risk of HTV/ AIDS and STDs were provided. She was educated related to proper nutrition for her pregnancy and discussed for rehabilitation for driving out.
Based on the above -mentioned circumstances, daily follow-up visits for a fortnight and an appointment with maternal health clinic for psychiatric support would be profoundly beneficial. Kindly encourage her to participate in methadone program alcoholics anonymous. Her first trimester ultrasound is scheduled in two weeks.
If your have any queries regarding Ms Wahlberg, please do not hesitate to contact me.
Yours sincerely,
Registered nurse.
