Mr Martin Wilson, who has recent episode of suicidal attempt, request prompt attention and follow-up care from your facility.
Mr Wilson is a retired book keeper who lives with wife and unemployed son. He smokes 40 cigarettes per day and is a regular social drinker. Gambling addiction has been initiated for the past two years following his retirement. As a result, he lost not only his friends but also lots of money and he is in crisis now.
On 13 October 2009, Mr Wilson was admitted with an excessive intake of Mogodal. On examination, he was weak and depressed due to gambling addiction. Therefore, antidepressants was commenced. Currently, he feels guilty and wants to overcome this addiction.
Based on the aforementioned circumstances, your urgent attention would be profoundly beneficial for Mr Wilson. Encouragement and assistance are recommended to join Gambling Addiction Action group as well as Quit line service. Decision shall, to motivate him to participate in social sporting activities such as lawn bowls as he used to play the same. I will chair is required for the first week followed by a frame. Kindly remind him regarding the monthly follow-up appointment is with Psychologist.
