Re: Ms Riya Roy, DOB:12Match 1998
I am writing to refer Ms Roy who needs supporting services in your’s facility.
On 21 February 2018, Mr Roy was admitted it 18:00 hours she had a normal vaginal delivery. Post-natally, Both the baby and their mother or haemodynamically stable. Upon discharge, babies weight was around 3.95 kg and feeding well.
Miss Roy lived with her boyfriend and separated. In 1.5 years ago she lost her parents in an accident. In case of emergency, his maternal aunty can be contacted and fourteenyears before, she had tontillectomy.
Would be extremely beneficial, if you provide a woman refuse shelter for her stay. Kindly arrange a single mother benefits and local maternal health services to her postnatal monitoring or recommended. Clinical psychologist referral is needed for her emotional support, regarding parents and boyfriend relationship dynamics. I endosed all necessary documents.
