Dear Ms Ward,

Re: Mrs Charlotte Prince, aged 86

Mrs Charlotte Prince, who is recuperating from mild concussion following a fall, requires home visits twice weekly, for further assistance and evaluation from your facility. She is scheduled to be discharged today.

During hospitalization, a series of investigations including a computed tomography of the head were performed, which showed no abnormalities. On assessment, the right was found infected due to untreated ingrown toenail. Therefore, in addition to her regular medication, amoxicillin 500 mg thrice daily for two weeks was recommended. Along with that, the wound was cleaned and dressed up adequately. Also, continuous supervision of neurological changes was provided.

Mrs Prince, a retired postal worker, is a widow and lives alone. She has little interest in meeting with the visitors and also with the general practitioner. She smokes 40 cigarettes daily and drinks occasionally. She doesn't prefer meals on wheels instead she cooks herself. Medical history of hypertension, since past 25 years and mild scoliosis has been noted; for which she takes ramipril 10 mg and Ibuprofen on requirement, respectively. Mobilization has been managed with a stick.

Based on the aforementioned circumstances, assistance with activities of daily living would be profoundly beneficial. Importance of handwashing, food hygiene and proper rest needs to be discussed. Monitoring for amoxicillin and ramipril is strongly recommended.
