Re: Mrs Patricia welshman,DOB:25/03/1930


I am referring to Mrs Patricia welshman into your facility, who requires full memory assessment and diagnosis owing to her early stage anheirmers and associated condition.


Mrs welshman,a widowed women is a long term patient of our facility who lives alone. On 19 June 2014, she had a fall and bruised her nose. For home assessment and daily activities were provided by occupational therapist and home care were supported by local council. Does not take the medication regularly and suggested for weheter pack to store the medication on the weakly basis.


On 19 April 2015, welshman's daughter wanted to discuss memory issues. She reported poor memory were observed in behavioural changes,and decision making issues. Due to poor memory,she does not attend social events,and in dinner engagements. On assessment,she is not oriented to date time and year. Has a family history of aliheimers disease and she was diagnosed as osteoporosis and dementia. She is on atiovastatin 20mg, ibuprofen 200mg,metoprolol 100mg, and paracetamol 500mg.


Based on the above mentioned circumstances, it would be appreciated if you could provide appropriate diagnosis and post assessment to Mrs welshman as you feel appropriate.
