Re: Mr Martin Wilson, aged 62
I am writing to refer Mr Martin Wilson who was attempted suicide, requires immediate care and support. He is scheduled for the discharge today.
On admission 13th October 2009,he was noted to be weak and depressed forwhich, he has been administered with lovan on 200 gm.In addition, he has the complaint of loss of appetite thus, his parameters revealed and confirmed his physical unfitness.
Mr Wilson is a retired bookkeeper,who lives with wife and two children in housing trust maisonette in Elizabeth. He has the habit of regular social drinking and heavy smoking and started gambling 2 years ago; however his gambling addiction leads to financial crisis and social isolation result in depression. Although, his wife and family irritated and distressed with his attempt he has the decision to overcome the situations.
Mr Wilson requires gambling addiction therapy in order to relieve from the situation,along with this your motivation and assistance will be needed to join on gambling addiction action group.Apart from these, encourage him to contact quitline for further clarifications.As he prefers to play lawn bowls involve him in the sports activities. Please note that, wheel chair should be provided for another week whereas, frame required after the psoriasis treatment.He has been scheduled with Dr Brian Psychologist in a monthly basis.
