Dear Ms Adams,
Re: Mr Martin Wilson, aged 62
Mr Martin Wilson, who attempted suicide on 13 October 2009 is being referred to you. He requires further assistance with involvement in gambling addiction program following his discharge today.
Mr Wilson was retired from Holden Car Company two years ago. He lives with his wife, Juan, who works at Coles and son who is unemployed. He is a heavy smoker and drinks daily. He loves playing lawn bowls. Currently, he is in a debt due to losing all the savings of pension in gambling. Consequently, suicidal attempt with overdose of magadol following depression was made. His family was unaware about gambling addiction.
At present, Mr Wilson is regretful upon his acts and wishes to control over the addiction of gambling. Along with that, poor - relationship with friends is noted.
Based on the aforementioned circumstances, involvement in Gambling Addiction Action Group would be profoundly beneficial. Education regarding quitline programs was provided. Follow - up appointment with Dr Brian Murphy is scheduled for the revaluation of depression. Liasing with sport activities has been recommended.
