Re: Ms Sonia calleja,aged 85
Thanks for accepting Ms Sonia calleja,who has been recuperating from right inguinant hernia and block repair. She requires atmost support and care following her discharge.
Ms calleja lives alone in proximal distance to her daughter. She has history of inguinal hernia. Initially, she was admitted with severe onset constipation bloating, abdominal pain, swelling and nausea. Therefore,an X-ray informer confirmed the diagnosis after which she successfully underwent surgery.on subsequent,month of her recovery was phenomenal. Hence referred to geriatric rehabilitation and assessment unit. Despite ambulation,she became frail associated with confusion and fecal incontinence.
Similarly, Ms calleja is unhappy and anxious after surgery and daughter cannot support her at home. An aged care assessment team assessed her and recommended for a permanent 24 hour nursing home. Eventually,she reluctantly agreed for the decision.
Upon referral,it would be profoundly beneficial, if you could support Ms calleja and her family for a transitioning to an aged care.
