Re: Mr Abdulla Gahtani,DOB: 5 June 1968
Mr Abdulla Gahtani, who has been recuperating from coronary artery bypass graft. He requires home visits from your facility following his discharged today.
On 7 August 2018, Mr Gahtani was admitted with severe chest pain that radiated towards jaw and left arm. Therefore,an X-ray chest and ECG were done which confirmed myocardial infraction. Following which he underwent surgery successfully. Postoperatively,she has been under ECG monitoring with prescribed medication. Additionally, physiotherapy has been commenced.
In terms of past medical and social history, Mr Gahtani is married resides in Australia but doesn't speak English. He is a smoker and his BMI falls under obese category.
Upon discharge, kindly monitor Mr Gahtani's blood pressure and heart rate during visits. Along with that educate him regarding the importance of smoking cessation and encourage him to perform exercises daily. It would be greatly appreciated if you could arrange a dictition for new diet plan and a social worker to register the family members with local, general practitioner. Please note, a follow-up appointment has been scheduled on 25 August 2018 at 2pm in outpatient department.
