Dear Mrs. Adison,
Re: Mrs. Margarat Adam, DOB: 08 May 1945
Mrs Margaret Adam, who underwent total left hip replacement surgery, request home visits for 2 weeks from your facility following her discharge today.
On 21 July 2021, Mrs Adam was admitted with a fractured left hip following a fall at home. Consequently, she has undergone the aforementioned surgery post-operative pain and constipation were managed with paracetamol and dulcelax respectively. Deltaparin 500014, Alexdronic acid 70mg, Calcium and Vitamin D were commenced.
Currently, Mrs. Adam has made good progress. A walking stick is needed for ambulation and she perform certain activities of daily living independently. Slight swelling on the hip region was noted; therefore, monitoring is Necessary for the same as well as wound and pressure ulcer. Additionally, supervision is a necessary for self-administering of Deltaparin which needs to be discontinued on 17 August 2021.
Mrs Adams, a widow who has no close relatives. She has a supportive neighbours.
Based on the aforementioned circumstances monitoring of medication compliance and encourage exercises programs are profoundly beneficial. An anthembolism stockings are to be kept until 6 September 2021. Wound dressing and medicine revival will be on 06 and 22 September 2021, respectively.
