Re: Mr Jamie Morgan, DOB: 22 February 1956
This letter is being written on behalf of Mr Jamie Morgan, who was diagnosed with coronary obstructive artery disease. He requires education on dietary modifications from your facility following his discharge today.
Mr Morgan, a fencing contractor, is single and lives alone. He smokes 30 cigarettes and drinks 3 bottles of beer on regular basis. His preferred food includes sausages, deep fried chips, burgers and eggs. Weight of 96 kg and height 177cm has been noted. Apart from that, he is allergic to peanuts.
On 27 September 2017, he was admitted with suggestive indicators of aforementioned disease. Consequently, coronary artery bypass with four grafts was performed. Postoperatively, conservative treatment was provided. Importance of smoking abstinence and decreased alcohol intake and consuming low fat diet was discussed. At present, an excellent progress has been developed. Incision site shows no complications, and mobilization has increased. In addition, he has been reviewed by the social worker.
Appointment with Dr Stephenson George, the local general practitioner has been scheduled on 10 October 2017 at 2 pm. Rehabilitation exercise regime, under local physiotherapist's supervision would be continued. It would be greatly appreciated if you could contact Mr Morgan at 9476 Old Dam Road Goondiwindi QLD 4390 to suggest a relevant diet according to his current condition.
