Re: Mrs Patricia Weishman, DOB: 28 March 1930
Mrs Patricia Weishman, who is suspected with early stage of Alzheimer's disease, is being referred under your expertise for complete memory evaluation and further treatment.
On today's assessment, poor short - term memory, for example recognising date and day as well as behavioral changes and difficulty in decision making were noted. Her daughter showed concerns, and has requested for further evaluation. Education regarding Alzheimer's and dementia, and their differences was provided.
Mrs Weishman has been receiving our services since 2007. On 19 June 2014, she presented with a bruised nose following a fall; consequently, review from occupational therapist and local council was recommended. On 14 December 2014, the blood investigations revealed low density lipoprotein of 2.9 mmol/L and vitamin D < 54. In addition, blood pressure was recorded as 145/85 mm/hg. Issues filling the spare script were identified; therefore, webster pack was provided. On consecutive visit, increased low density lipoprotein and decreased vitamin D level were reported. Mrs Weishman, a widow, lives alone. Her daughter, Christine resides at distance of 10km. She follows a course of medication, including OsteVit - D 1000 IU, Lipitor 20mg in the morning, Metrol 100 mg twice daily along with Brufen 200 mg and Panadol 500 mg on requirement, for osteoporosis.
