I am writing to provide information regarding your today's visit to Stillwater NHW walk-in centre. This letter will give you comprehensive details that probably help you to seek your Gp's medical attention.


On 8th February 2010 you visited our centre with a badly scratched right arm which was happened seven days ago while playing with your family cat. However,you have been regular to school and not too worried about the stracth. Unfortunately,the stratch appeared red with puss formation which stung at times. In addition,you reported headache, stomach cramping,chills at night and ache and dull right arm pain which radiating towards right armpit. ( On assessment your vitals were unstable especially blood pressure was quite abnormal for your age. Consequently, swollen lymph nodes at right armpit region was noticed. Therefore,you has been diagnosed with cat scratch disease.


As you recall,hear pad applied in scratched area for pain relief and cleansed with savlon. Educated to use antibacterial soap to keep scratched area out of infection. Eventually,nurse practitioners prescribed Ciprofloxacin 500mg thrice daily for 10 days.


If you notice any additional symptoms, please return to walk-in centre or contact your GP.
