Re: ms Sheila Cartwright,aged 69


Mrs Sheila Cartwright,who has been recuperating from unstable diabetes mellitus and left foot ulcer. She requires continued care from your facility following discharge.


On 14 April 2017, Mrs Cartwright was admitted for streamlining non-insulin dependent diabetes mellitus for which she has been on hypoglycemic agents and oral antibiotic therapy for infected foot ulcer. However,she has periodic problem with self- administration of medication associated with boozing and poor diet pattern. Therefore, adviced the continue hypoglycemic medication. Apart from that saline dressing has been applied daily in addition to antibiotic therapy which has to be reviewed with wound healing progress.


Upon discharge, kindly monitor her blood sugars, alcohol intake and dry compliance. As Cartwright is retired,she might be encouraged to undertake some income producing activity and way to expand social activities. A referral to community social worker may be of benefit.
