Dear Ms Gold,


Re:Mr George Gail, DOB: 24 April 1936


I am referring Mr George Gale,who has been diagnosed with urinary tract infection, requires continuing care and expedient management at your service following his discharge today.


On 10 May 2021, Mr Gail admitted to Oldtown Hospital with the complaints of fever and disorientation secondary to a fall. During hospitalization, he was treated with intravenous antibiotics and amoxicillin 750mg thrice daily. His urinalysis confirmed the diagnosis. Currently, he has made a good progress;however, episodes of confusion is still exhibited.


Mr Gail, a widower, lives alone in a level 2 flat without lift.His son lives abroad and has no family nearby. He has had hypertension and gastro esophageal reflux disease since 2009 and 2013 respectively, for which he is on Felodipine 5 mg daily and antacid tablets. He also has osteoarthritis and non specific colitis and is on paracetamol 500mg four times daily.


Pertaining to the above details, it would be profoundly beneficial if you could evaluate Mr Gale for independent living capacity. Although significant reduced mobility is detected, he is eager to return home as early as possible. Kindly encourage physical mobility at least 30 minute daily.
