RE: Mr Lionel Ramamurthy,

Aged 63 years


Dear Ms Ponsford,


Mr Ramamurthy, who was admitted into our facility a week ago has been treated for Pneumonia, requires assistance and continue monitoring back into your care following his discharge tomorrow.


On admission, shortness of breath, chest pain, fever and rigors were observed and treated. He is afebrile now; inflammatory markers are back to normal. Though he can shower and toilet himself and walk in dependently, but he is need observation and assistance as slow speed is noticed. According to dietician review, 1.5 KGS weight has been gained; therefore, diet monitoring has been advised, in order to maintain good nutrition. Chest physio, deep breathing and cough exercises are needed to be encouraged. In addition to this, sitting position is preferred, in lying position, postal drainage needs to be ensured.


Overall, he has made good progress. Tablet paracetamol needs to be given if required for chest or abdominal pain. kindly keep him warm. It would be appreciated if you can continue his care and follow above mentioned advises.
