Re: Mr Christopher Fort, aged 49
I am writing to request a home visit for Mr Christopher Fort, who has been recuperating from post coronary angioplasty. He requires monitor including medication adherence and dietary regimen, as he is scheduled for the discharge tomorrow.
On admission 20 February 2020, Mr Fort was presented with complaints of chest pain radiates to shoulder, breathing difficulty along with generalized weakness, nausea, dizziness since 2 days. On examination, ECG reveals ST elevation and positive to ckmb,it declared the diagnosis of myocardial infarction.
Mr Fort, has underwent an angiogram on 21st February 2020, it reveals blood clots in the left side of the coronary artery, forwhich performed above-mentioned surgery. Consequently, he has made a significant progress in the postoperative period.He has been instructed to avoid oily foods and carbohydrates and add the adequate quantity of green leafy vegetables and fruits in his diet pattern.
Mr Fort, had history of peptic ulcer therefore, clipping was performed twice. In addition, he has had hypertension since 8 years and diabetes since 9 years owing to this,he is on tablet nicardia 5 mg once and injection human mixtard twice daily; however,he has been noted to have minimal physical activity.
It would be great beneficial, if you could monitor Mr Fort's vital signs, medication compliance along with dietary management. Medication chart is enclosed with this letter for better clarification. Although, he requires daily exercise, preclude him in sternous exercise at least for two weeks. Make arrangements to join some quit-line programs to improve his healthy life. Please note that,he has been scheduled for the followup on 3rd March 2020 with the reports of FBS and FLP.
Based on the aforementioned circumstances,your guidance is profoundly beneficial for Mr Fort to lead a healthy life. In case require additional information,please do not hesitate to contact me.
