Dear Sir/Madam,

Re: Ms Riya Roy, DOB:12 March

1998

I am referring Ms Roy , who is a postnatal mother requires, emergency short term admission for accommodation and other support services, following her discharge today.

On 21 February 2018 , she was admitted for delivery and on the following day , she delivered a female healthy baby without any complications. Postnatal progress was excellent with minimal blood loss and her hemoglobin level is 135g/L.Ms Roy prefers the baby to bottle feed rather than breastfeeding. Baby’s feeding is noted to be good and normal.

Ms Roy is single, used to live with her boyfriend but currently refuses to reside with him as he is verbally abusive and even not the father of the child. Presently she is unemployed and has no aces to stay with her family as her only sister stays overseas and parents died a couple of years ago with an accident.

After that incident, she went into depression and left untreated for which a referral to clinical psychologist was done for emotional support as well as for counseling regarding her relationship with her boyfriend.

Brochures and suggestions were given to apply for single mother benefits and also requested local mental health services to provide ongoing postnatal monitoring and care .

Based on the above mentioned details, it would be greatly beneficial if you could provide accommodation and other required services until she get further support for her living.

If you have any queries, please do not hesitate to contact me.

Yours faithfully
