Dear Nuse,

Re: Mr Alfred Billy, aged 52

I am reffering Mr Billy, who was treated for basal cell carcinoma in neck, requires continuing care and support following his discharge today.

On 21 Maarch 2010, Mr Billy was admitted and underwent surgery. Postoperatively, skin biopsy has taken for pathological study. On assessn slight wound infection, breakdown and reactionsin suture area were recorded, probably owing to incomplete excision.Pandein forte was administerd for pain management.Currently, his wound is better and has no constant swelling. He is fit for discharge today.

Mr Billy has been drinking alcohol for the past 30 years and smokes cigratte. He lives with his wife and works as a truck driver.

It would be highly appreciated, if you could provide regular monitoring for Mr Billy. Please note, his medicines needs to be continued 7 more days.

Please do not hesitate to contact me for further queries.

Yours sincerely,
