Dear Sir/Madam,
Re: Baby Alicia Nesbitt, age: 6 days
Baby Alicia, who was diagnosed with a low - birth weight and effects of opioids due to maternal drug abuse, requires daily home visits till full recovery, from your facility following a discharge today.
Baby Alicia was born with weight of 2.0 kg. Poor appetite, probably due to improper bottle feeding was noted. Imbalaced bonding between mother and baby was recorded. Mother required prompting to care the baby. Ongoing substance abuse was managed by the drug and alcohol team.
Her Mother Gemma, 20 - years - old, is unmarried and works as a sex worker. Heroin abuse for past two years was recorded. During pregnancy, domestic violence by Alicia's father resulted in broken relationship. Gemma's parents and grandmother are supportive to her.
Currently, no complications post - drug withdrawal has been identified in mother and baby.They have been scheduled for the discharge today.
Based on the aforementioned circumstances, supervising the health progress of Alicia would be profoundly beneficial. Social worker needs to be informed in case of any risk arises for the baby. Drug and alcohol team assistance is recommended to monitor the after effects of heroin abstinence. Observation for mother's behaviour and mental status would be required, and her mother and grandmother needs to be educated about caring the baby.
