Dear Director,

Re: Ms Riya Roy, DOB: 12 March 1998

Ms Riya Roy, who delivered a female baby on 22 February 2018, requires a temporary residence and assistance following her discharge today.

On 21 February 2018, she was admitted to our hospital at 06:00 pm with suggestive indicators of labour. Next day around 12:30 hours, the baby was delivered vaginally with 3.77 kg weight. Post - delivery, blood reports showed hemoglobin of 13.5 gm/l. Bottle feeding was preferred due to lack of knowledge regarding breastfeeding techniques. Currently, the baby is feeding properly and her weight is 3.95kg.

Ms Riya is unmarried. Since August 2017, she resides with her boyfriend, who abuses verbally for last 3 months; therefore, she is not interested to live with him. Father of the child lives in Europe and has no contact with her. In addition, she has depression since her parent's death and no treatment has been received yet.

Based on the aforementioned circumstances, postnatal supervision from Maternal Health Service would be profoundly beneficial. Appointment with clinical psychologist for ongoing mental support and counselling regarding the relationship issues has been scheduled. Education on benefits of single mother has been provided.

In case additional information is required, please do not hesitate to contact me.

Yours faithfully,

Registered Nurse Maternity Ward Women's College
