Re: Ms Olivia jack,DOB: 04 may 2000
Ms olivia jack who delivered her first child on 10 June 2019 through emergency caegorean section,requires assistance and a home visit for futher management and councilling.
On 09 June 2019, Ms Jack was admitted to hospital with foetal distress and failure to progress, therefore,she underwent the surgery she delivered a female baby with birth weight of 3.9 kg. She lost 200ml of blood during post portom haemorrhage. Her haemoglobin was 90g/t,for which iron and vitamin C is recommended. She prefers bottle feeding as she has no confidence in Brest feeding and caring for baby including bathing.
Ms Jack lives in a rental shared accomodation and recieves role parent benefits. She has no contact with baby's father for the last nine months including her family members. She feels insecure and isolated and wants to offer her baby for adoption as she is not confident in her ability to be a good mother.
