Dear Mrs Penny,
Re: Ms. Sheila Catwright, aged 69
I am writing to introduce Ms Sheila Catwright, who us diagnosed with unstable diabetes mellitus and a left foot ulcer. She requires follow-up and is being discharged today.
On 14 April 2017, Ms Catwright was admitted to hospital to manage non-insulin diabetes mellitus, for which she has been treated with oral hyperglycaemic agents, for the past 9 years. In addition, had a infected foot ulcer at left leg, for which has been commenced an antibiotic therapy with daily saline dressing. Antibiotic need to be reviewed at the completion of the current course. She requires ongoing management for medication complaince, blood sugar levels and to monitor the wound for signs of complications.
Ms Catwright, a retired woman who resides alone. Since retirement, increased alcohol intake and reduced dictory pattern has been observed. Please help her to introduce the establishment of social activities as well as to engage in moderate exercises. Her income has reduced , since retirement might be encouraged to involve in income producing activity. Monitor her dictary pattern and consultation of alcohol. A referral to community social service worker is needed.
