Re: Mr Jaime Morgan,DOB: 22/07/1956


The purpose of this letter is to selicit low fat dietary guidelines for Mr Morgan who presented to us on 27/09/2017,was diagnosed with obstructive coronary artery disease,and underwent a coronary artery bypass graft under our care. He is being discharged today.


His risk factors include being over weight,due to a diet primarily consisting of fat rich foods like sausages,pasta,burgers,eggs and deep fried chiks. His height is 177cm and he currently weights 96 kg furthermore,he is a smoker and smokes atleast 30 cigarettes a day. Apart from that, he drinks beer 300ml bottles of beer regularly. He is allergic to peanuts.


Mr Morgan has progressed well after the surgery. He has been educated regarding smoking cessation and reducing of alcohol intake. He has been advised to loose weight through exercise and diet, therefore,he has required a detailed advice on simple low fat recipes that can be easily prepared at home.
