Re:Mavis Brampton, aged 72


I am referring Mr Mavis Brampton, who has been recovering from pleurisy. She requires ongoing care and monitoring after being discharged on 15 January 2011.


On 10 January 2011, Mr Brampton was presented with the complaints of breathlessness and productive cough. High BP slightly raised temperature and overweight with BMI 29 have been noticed. Consequently, he has been diagnosed with pleurisy, revealed by chest X-ray. Therefore, he had been treated with O2 supplementation, IV amoxycillin and provastatin 20 mg per day along with celecoxib.


Mr Brampton has been widowed since 25 years. In addition, he has been smoking since teenage. Currently, he smokes 10 cigarettes per day.


During hospitalization, nicotine patches and zyban 150mg 2 times per day has been advised to cease smoking and practicing deep breathing exercises for breathing difficulties. Upon discharge, it would be appreciated if you could monitor his quit-smoking plans and medication compliance. In addition, provide him assistance regarding nutritious diet and in house keeping. Please note, zyban and nicotine to cease as soon as craving for cigarettes has stopped. In case of any side effects of zyban, it can stopped immediately.
