Re: Ms Patricia Styles, aged 62
I am referring Ms Patricia Styles, who has been suspected with post pericarditis complications during homevisit.She requires immediate assessment and further management.
On 30 August 2019 , during homevisit Ms Styles was presented with chest pain, shortness of breath and fatigue.On examination,her vital signs shows low grade fever and tachycardia. Consequently, she has been annoyed with her health condition; however her blood glucose monitoring and medication compliance has been up to date.
On 23rd August 2019, she was admitted in Green Valley Hospital with pleuritic chest pain and other related symptoms therefore, lab investigations, chest x-ray and echocardiogram was performed.The reports revealed viral influenzae type b plus pericarditis owing to which, she was treated with intravenous fluids and ibuprofen 600mg thrice daily.After the treatment, she showed a significant improvement in her condition and discharged with followup home visit on 30th August 2019.
Ms Styles, is a retired school teacher and who lives alone. Her husband expired due to lung cancer and has no children. She has the habit of drinking one to two glasses of wine once in a week.She has had hypertension,type 2 diabetes and has been diagnosed with depression on June 2016 following the grief of her husband's death. She is on quinapril 40 mg, metformin 500 mg twice daily and APO-Gliclazide MR 30 mg once daily.
