Re: Mr George Gale, aged 85


I am referring Mr George Gale, who has been recuperating from urinary tract infection.He requires temporary care and management of your facility,as he is scheduled for the discharge today.


Mr Gale, was brought to the hospital on 10 May 2021 following a fall at home. On examination,he was confused and disoriented. is vital signs and urine analysis shows elevated for which he was treated with intravenous antibiotics amoxicillin 750 mg thrice daily. He had single episode of vomiting,palpitations and dysuria 2 weeks ago.


At present, Mr Gale has made excellent progress; however he has been facing episodes of confusion therefore, requires assessment for independent living. In addition he has difficulty in mobilizing forwhich he needs 30 minutes daily physical activity.


Mr Gale, has osteoarthritis since 2003, hypertension since 2009 thus, paracetamol was administered 500 mg 2 tablets 4 times a day and felodipine 5 mg once daily. In addition,he has had non-specific colitis since 2019 which requires ongoing monitoring.He is a retired retail manager and widower, who lives alone in his own flat.He is a social drinker and smoker.


Based on the aforementioned circumstances, requires assessment and provide necessary management to achieve the optimum level of self care before he return home. In case any additional information, is required please do not hesitate to contact me.
