Re:Mrs Joy Rafferty,aged 65
Mrs Rafferty who was diagnosed with essential hypertension and mild depression ,would requires your ongoing support and monitoring, for improved quality of life.
Mrs Rafferty has history of undifferentiated osteoarthritis with aches and pain,she has been given analgesics for pain.although she has been diagnosed benign hypertension for more than 10years but she was on coversyl 4mg daily.also for the past year she start experiencing increased in weight from 75kg to 90kg and was adviced on exercise.
Mrs Rafferty has retired 10years ago,a widow with 6 adult children ,and 3 are living nearby in the same compound. She has been in support of the family since death of husband.due to some factors Mrs Rafferty was restricted to driving, attending worship places,and family activities was restricted only on occasions.she reported feeling of osolation,in view of that family members was involved to encourage her on social activities so as to prevent boredom and decrease family dependence.
It would be greatly appreciated if you could help in improving general health,monitoring of blood pressure regularly and engaging her to participate on exercises regime,joining of activity group around the area and involving family care would be beneficial.
