Re:Mr sean William, aged 72
Mr Sean Williams, who requires health education regarding inhaler use as his condition deteriorated due to inappropriate use of inhaler.
Mr Williams is a widower, who lives alone. He has chronic obstructive pulmonary disease and osteoarthritis for which he takes salbutamol ipratropium 25/250 inhaler two puffs everyday. In addition, he has a leg ulcer and daily dressing has been provided from our facility.
06 February 2019,during home visit, Mr Williams complaint of increased shortness of breath while doing activities;although, get relief at rest. On assessment, inhaler use was not appropriateband it lead to bronchospasm. Therefore, he was advised to ceased the inhaler and started ventolin HFA teo puffs under supervision.
Today, during revisit, he has persistent complaint and ventolin inhaler was not alleviate the symptoms.
Based on the aforementioned circumstances, it would be appreciated if you could educate Mr Williams regarding the inhaler dosage and how to use it effectively.
