Dear Sir/Madam,

Re:Ms Olivia Jack, aged 19

Ms Olivia Jack who had an emergency caesarean section, requires advise and assistance following discharge today.

Ms Jack is a single mother, who lives in a rented shared flat with another woman. She receives sole parent benefits. She feels isolated and insecure also thinks pregnancy was unwanted. Expressed concerns regarding adaptation as she os not confident about her ability to be agood mother.

On 19 June 2019,Ms Jack was admitted to hospital during the first stage of labour. Emergency surgery was done due to foetal distress and failure to progress the delivery.Female baby was born on 10 june 2019 with birth weight 3.9kg. Post partum haemorrhage was treated with fefol vit spansule and vitamin c. Presently, her condition has improved, hemoglobin is 90g/L, and wound is healing well. Breast feeding initiated;however, she is not confident and prefer bottle feeding. Im addition, she is not confident in bathing and caring for baby.

Based on the aforementioned circumstances, it would be extremely appreciated if you could advise her regarding importance of breast feeding and kindly assist her for caring baby untill she is confident.

In case of any further information required, please do not hesitate to contact me.

Yours faithfully,

Registered Nurse

Mother Hospital

South Brisbane.
