Re:Robyn Harwood, DOB :4 February 1950


I am referring Ms Robyn Harwood, who has been recuperating from right partial rotator cuff tear. She requires monitoring and assistance at your facility after being discharged today.


On 30 June 2011,Ms Harwood has had a fall and slipped onto outstretched arm. Based on this, he had been undergone X-ray and MRI which was revealed partial rotator cuff tear. She has been experiencing pain and weakness while lifting arm overhead for which daily physiotherapy has been initiated.


Ms Harwood has been suffering from type 2 diabetes mellitus, for which metformin 500mg in the morning has been prescribed. In addition, Ms Harwood is a widow, lives alone. However, she has no relatives except a niece who lives in Sydney with her husband.


Ms Harwood had refused surgery after a discussion with the orthopedic surgeon. she has been decided to stay on non-surgical treatment. Upon referral, it would be greatly appreciated if you could arrange home visits for monitoring blood glucose levels fourth hourly, assisst her in showering and household work. Please note, she has been scheduled for orthopedic rewiew on 1 August 2011.
