Re:Mrs Larissa Zaneeta, aged 38


I am referring Mrs Zaneeta, who has been experiencing failure to conception. She requires further assessment and advice at your facility.


On 11 July 2005,Mrs Zaneeta was presented to gynaecologist with the plan of conception in 12 months. She has been complaining of tiredness and sleeping difficulty due to work stress. On examination, pale appearance, restless and BP 140/90 have been noticed. As a result, she has been diagnosed with stress-related anxiety, for which she had been advised to reduce work hours, relaxation techniques and sleeping pills.


Regarding her next visit on 15 August 2006, she has been worried related to her conception even after stopping the oral contraceptives and in menstruating condition. Based on this, she has been educated regarding frequency of sexual intercourse, methods, temperature and cycle. In addition, 1-2 Valium twice a day has been prescribed after undergoing pelvic exam and pap smear.


Regarding her husband's sexuality, normal libido, no erectile dysfunction and normal sperm count have been noticed. Upon referral, it would be appreciated if you could do further investigations and monitoring related to Mrs Zaneeta's conception.
