Dear Psychiatrists,


Re:Ms Carolina Ayuso, aged 25


I am referring Ms Carolina Ayuso, 25-year-old who has been recuperating from paracetamol poisoning. She requires ongoing care and further assessment after being discharged tomorrow.


On 1 December 2018, Ms Ayuso presented with the symptoms of slurred speech, headache and diaphoreses followed by consumption of 20 paracetamol tablets after having 1 bottle of wine. On assessment, he has been responded to verbal commands. However, drowsy, pale appearance and 2 episodes of vomiting have been reported. Based on this, she had been admitted into hospital for observation and investigations has been sent. In addition, administered antidote N acetylcystene in Ns and vitals, GCS had been monitored hourly.


Pertaining to social background, Ms Ayuso is unmarried, works as an accountant and lives with mother.


On today's report, although she has gained consciousness and subsided vomitings, she has been appearing pale and drowsy. IV administration had been stopped and she is able to take liquid and soft diet. However, she seems to be very detached.she had been advised for discharge tomorrow in case of investigations reports are normal.Upon referral, it would be appreciated if you could do further assessment and arrange a review with toxicologist.
