Dear Ms DiCoccio,
Ms Bethany Tailor, who was diagnosed with schizophrenia, requires ongoing evaluation and management following the discharge back to apartment today, from your facility.
On 1 March 2018, she self - admitted with suggestive indicators of decompensated schizophrenia, probably due to medication non - compliance and substance abuse. Auditory and visual hallucinations as well as delusions along with agitated and aggressive behaviour were reported. Significant improvement was noted postprescribing risperidone. Regular assessment for symptoms of psychosis, and redirection from delusions were implemented. Medication - compliance was strictly monitored, and behavioral control therapy was initiated.
Ms Tailor resides with her parents and has marked history of hypertension secondary to fibromuscular dysplasia, primary hypothyroidism and polysubstance abuse, primarily cocaine and alcohol. She has been receiving schizophrenia disability pension.
Her condition may exacerbate in case of non - compliance to the medication and drug abuse. It would be greatly appreciated if you could supervise Ms Tailor for resperidone 4gm daily at night and 1mg twice daily if agitation or psychosis persists.
