Re: Ms Monica Riss, aged 26


I am writing to refer Ms Ross who requires your assessment and continued care for her withdrawal symptoms after her discharge tomorrow.


On 03 May 2019, Ms Ross was brought to the hospital following an attempt of self harm with drug overdose. She had consumed 20 tablets of paracetamol along with a bottle of wine. After medical intervention and management with the antidote, Ms Ross has recovered we'll in the last two days. She still appears pale and drowsy ; however, she is conscious and her vitals are stable.

Ms Ross is single and lives with her mother who is an aged pensioner. She has a sister and her father had passed away 8 yea


rs ago due to heart disease. Ms Ross used to play sports but has shown less interest now. Although, she used to see her friends on the weekends. Ms Ross does not have any previous history of the drug overdose or mental issues. She is not on any medication.


It would be greatly appreciated if you could take over of Ms Ross and provide continued care. Apart from this, her results of the latest investigations are being awaited, and group review by sociologist is also scheduled.
