Re:Akira Annaisha, aged 5


Akira Annaisha, who has been diagnosed with acute meningoencephalitis, requires follow-up care and health education following discharge today.


15 July 2019 Akira was admitted to the emergency department and diagnosed with acute meningoencephalitis due to complication of mumps. Her condition was managed with conservative treatment.presently, her condition has improved.


Akira lives with her parentsand two siblings. They are migrated from Japan, and they understand limited english. As per medical history, childrens were vaccinated at the time of birth;however, records are not available.


Inthe view of above, it would be extremely appreciated if you could educate the parents regarding the importantce of vaccination and record as they are unaware of immunisation schedule. Monthly follow-up appointment recommended for neurological check-up. Kindly monitor the compliance with vaccination schedule.
