Re:Ms Bethany Tailor, aged 35
Ms Bethany Tailor, who has been diagnosed with schizophrenia, requires rehabilitatitive care from your facility following her discharge today.
On 01 March 2018,Ms Tailor admitted with complaints of auditory and visual hallucinations along with delusion. During hospitalisation, she was agitated, aggressive and responding to internal stimuli;therefore, she has been commenced on rispoderone and she was not compliant with medication. Following days, improvement has noted, able to minimize delusions and focus on activities of daily living.
Ms Tailor is a unemployes women, who receives disability allowance for schizophrenia. She has a history of polysubstance abuse, mainly cocaine and alcohol. In addition,she has hypertension secondary to fibromuscular dysplasia and primary hypothyroidism for which she takes levothyroxine 88mcg daily.
Based on the aforementioned circumstances, it would be extremely appreciated if you could monitor her medication adherence. Kindly assess her for any signs of psychosis. Aid her to maintain behavioural control and provide therapy if possible.
