Re: Mr Andy Williams, aged 65
Height: 183 centimeters, Weight: 155.5 Kilograms, BMI: 46.6 Kiligram/m3
I am referring Mr Williams, who has been diagnosed with obesity. He requires further assessment and management at your centre.
On 15 June 2018, Mr Williams presented to the clinic with the symptoms associated with obesity. After the assessment, the diagnosis has been confirmed. He has a history of childhood obesity and has been increasing the bodyweight at every decade; therefore, he was engaged in a commercial and the medical weight-loss program. Consequently, the recapturing the weight with in months was achieved. As a result, he refrained from the programs and was reviewed by a dietician. In addition, bariatric surgery was advised but he refused the surgical opinion due to the further complications.
Mr William, who is a radiologist and has a family history of obesity. He has had type ll diabetes mellitus, hypertension, and dyslipidemia, for which is on 30 and 70 units of NPH insulin before breakfast and dinner, tablet metformin 850 mg twice daily, tablet lisinopril and nifedipine, as well as tablet atorvastatin 10 mg. He consumes a heavy dinner at the night time.
In the view of the above-mentioned information, kindly provide him an expert evaluation and further treatment. Advice about the surgical options and improvement of his quality of life would be profoundly beneficial.
