Re: Ms Patricia Styles, aged 62
MS Patricia styles, who was admitted with viral influenza type B and pericarditis to Green Valley Hospital is being referred into your service. She requires urgent evaluation and management for suspected relapse of pericarditis.
During today's home visit, chest pain in lying position, shortness of breath and fatigue were observed. Temperature 38.1°C, heart rate 115, respiratory rate 28 and blood pressure 125/78 were noted.
On 23 August 2019, Ms Styles visited to meet her sister and was hospitalized following a sharp and pleuritic chest pain along with tachycardia, fever and malaise. Viral influenza type B and pericarditis were revealed by throat swab culture and echocardiogram respectively. Conservative treatment was provided. She was discharged on 25 August 2019 followed by advice on self assessment at home. Her niece drove to home and agreed to stay for 3 nights.
Ms Styles, a retired primary school teacher is a widow and has no children. Hypertension, type 2 diabetes mellitus and depression were diagnosed in 2011, 2013 and 2016 respectively. Her condition has been managed with quinapril, metformin plus gliclazide and and regular counselling. Usual blood pressure reading is 145/90. Apart from that, she consumes 1-2 glasses wine weekly, and walks around 20 minutes daily with her dog. Well-balanced diet has been maintained with the help of counseling with dietitian.
Based on the aforementioned circumstances, arranging an urgent assessment and treatment would be profoundly beneficial.
