Dear Ms Howard,
Re: Ms Betsie Anderson, aged 89
Ms Anderson, who is recuperating from urinary tract infection and confusion requires ongoing care and further management following discharge.
On 16 February 2020, she was admitted with complaints of decreased consciousness, fever, loss of weight, drowsiness and with urinary incontinence . The ED assessment showed increased temperature 38.3C, raised heart rate and respiratory rate. Required lab investigations were done which showed increase in wbc count and the results of urine and blood cultures were awaited. The condition was diagnosed as Urinary tract infection for which IV antibiotic was commenced .
Oral antibiotic Ciprofloxacin 250 mg twice daily for 14 days to be continued and her regular medications levothyroxine 200mcg, Protonix 75mg,Colace 100mg every morning along with multivitamin and Aricept 10mg at night also to be followed.
Ms Anderson general condition was improved gradually and ready for discharge. She was fully oriented and alert ; however, there is required assistance for mobilization and while during her activities of daily living.
Based on the above mentioned details, it is extremely necessary to monitor daily vital signs and level of consciousness. Continuing observation for any signs of dehydration as well as orthostatic blood pressure is also required. There is follow up appointment with GP Dr. Alex Rose along with lab investigations after 2 weeks .
