Re:Mrs Margarat Adam, DOB:8 May 1945
I am referring Mrs Margarat Adam, who has been recuperating from left total hip replacement surgery. She requires ongoing care and further assistance from your facility after being discharged today.
On 21 July 2021, Mrs Adam was complait of fall while walking. Her X-ray revealed a fracture of left hip acetabulum, for which she underwent left total hip replacement surgery. Post-operatively, she has been disoriented for 5 days. In addition, pain has been managed by paracetamol 1 gm ang dulcolax 10 ml for constipation along with this , anticoagulant and regular medications of calcium and vitamin has been commenced.
Mrs Adam has a history of multiple falls, which results in ankle fracture (2012) and wrist fracture (2018) as well as this, she has had osteoporosis since last year.
During hospitalization, mobility has been improved as a result of physiotherapy assistance. Re-dressing has been done and encouraged for Self-administration of deltaparin under supervision. Upon discharge, it would be appreciated if you could arrange home visits for her upto 2 weeks in order to monitor her condition and medication compliance. In addition, encourage her for exercise and stop administration of deltaparin on 17 August. Please note, she has been scheduled for re-dressing and anti-embolism stocking on 6 September and kindly arrange a medicine review on 22 September 2021.
