Dear Ms Chuang,
Re: Mr David Felix, aged 23years
I am referring Mr David Felix, who has been recuperating from pelvic fracture and suspected to have neurological damage forwhich, he requires continued care and psychological support from your rehabilitation centre.
On 6th April 2007,he was presented with broken neck and fractured pelvis followed by a car accident. He was treated accordingly and managed well therefore,he showed excellent progress. At a time, he has been started to use wheelchair.
Mr Felix, is a single and accommodate in a flat; however,parents are willing to take care of him when he returnto home.
Eventhough, he attained a good recovery, he requires close monitor in his care.It would be beneficial, if you arrange physiotherapy once daily, hydrotherapy twice a week and speech therapy thrice a week. Encourage mobilization, to improve blood circulation and quick healing of bedsore wounds. Kindly communicate with the university to continue his education and ensure him to join the social groups to combine with the similar age people. Please make arrangements to receive disability pension.
