Re: Mrs Jessica Christ

I am writing to refer Mrs Jessica Christ who underwent percutaneous coronary intervention. She requires rehabilitative care and further management.
Mrs Christ was admitted to the emergency department on 06 November 2021 with the complaints of chest pain, sweating and shortness of breath along with pain in the right shoulder. On assessment, her vital signs were remarkable. A series of laboratory investigations and radiology results revealed block in her right coronary artery; therefore, two stents were inserted.
Mrs Christ's post-procedural period was uneventful. On examination, significant increase of LDL, cholesterol, triglycerides, and troponin-I were noted. She was administered medications.
Mrs Christ is a widow. She has had type-2 diabetes mellitus since 2018, for which she takes Metformin 850mg BD. She does not perform any physical activity. Of note, she is allergic to Codeine.
It would be appreciated if you could encourage Mrs Christ to perform regular exercise. Kindly educate her regarding the importance of physical activities. Please note that a list of medication has been enclosed here with this letter.
