Dear Doctor,
Re:Mrs Priya Sharma, aged 60
I am referring Mrs Priya Sharma, who has been suffering from high blood glucose levels. She requires ongoing assessment and further management from your facility.
Mrs Sharma has had diabetes since 1994, for which metformin 500 mg two times at night and Glipizide 5 mg in morning has been prescribed. On 29 December 2013, blood sugars are 6-18 have been reported. In addition,steady weight with BMI 24 and blood pressure 155/100 have been noticed. Based on this, pathology has been sent including HbA1c and scheduled for rewiew after 2 weeks.
On rewiew after 2 weeks, pathology reports revealed poor control on sugars, for which changes in routine medications have been recommended . Metformin two times every night to once in the morning and evening along with this, lipitor 20 mg in the morning has been commenced. In addition, sugars have been improved and advised to re-sent the pathology for next visit.
On today's visit ,pathology reports revealed high range of fasting sugars. Pertaining to the above mentioned circumstances, it would be greatly appreciated if you could proceeds her diabetic care with further management.
