Re: Mr Lionel Ramamurthy, aged 63
Mr Lionel Ramamurthy who is recuperating from pneumonia will be discharged back into your facility. He requires further care and assessment following his discharge tomorrow.
Upon admission, acute shortness of breath, inspiratory and expiratory wheezing, chest and abdominal pain due to persistent cough as well as fever, rigors and generalized ache were noted. Mobilization was possible with the aid of pick-up frame; however, short distances were covered with increased shortness of breath on exertion.
During hospitalization, inflammatory markers returned to normal limits. Showering and toileting were performed slowly, but without assistance. Chest and abdominal pain was reduced. Weight of 1.5 kg was gained after the review of dietitian. Ambulation was performed under the supervision of physiotherapist. Education regarding intake of oral fluids and proper nutrition was given. Importance of deep breathing and coughing exercises was discussed. For ensuring postural drainage, sitting position was recommended. Currently, an excellence recovery has been attained.
Based on the aforementioned circumstances, paracetamol for chest and abdominal pain would be profoundly beneficial. Keeping the body warm should be advised. Monitoring for balanced and healthy diet including adequate amount of fluids, eggs, fruits and vegetables should be done.
